For your first visit I request that you come to your appointment at least 10 minutes early or if you would like to fill out a new client information form before coming to your first appointment click on New Client Form.
I do not file insurance but I would be happy to provide you with a receipt that lists the necessary codes and my tax ID# that you may file with your insurance company.
You must have a prescription from your Doctor or Chiropractor that prescribes Massage or Myofascial Release Therapy. This prescription will list a diagnosis that will be needed by your insurance company.
Just copy the prescription and the receipt from Therapeutic Massage and file with your insurance company for any reimbursement.
Your appointment is scheduled specifically for you. Lost appointments do cost massage therapists real dollars as we juggle very complex client schedules and advanced requests for appointments.
Please give 24 hours notice for cancellations, this allows me to schedule waiting clients. Except in emergency, the full session fee will be charged for late cancellations.
In order for me to uphold my professional standard of being "on time" I regret that I cannot give you additional time if you arrive late for your appointment.
If for any reason I am late starting your appointment, you will receive the full scheduled time.
If you have a scheduled appointment and you do not come in for your appointment and do not call you will be charged for the full cost of that appointment.
